Important Reasons to Call Avycare

Because we are committed to excellence in patient care, we want you to feel comfortable calling us with any patient issues or concerns that you observe.

For example, the patient:

a. Wants to call 911 for any reason.

b. Is planning to go out of town.

c. Has uncontrolled pain.

d. Receives a new medication.

e. Had no bowel movement in 3 days.

f. Is more short of breath than usual.

g. Has fallen: even if he/she can get up and thinks he/she is not hurt.

h. Is bleeding from any location.

i. Has nausea, vomiting, or diarrhea.

j. Experiences confusion, agitation, anxiety, sleeplessness or seizures.

k. Has a question or message for one of the care team members.

l. Needs to CANCEL a scheduled visit.

m. Needs to discuss a problem with a care team member.

n. Needs medication refills, medical supplies or medical equipment that you are unable to arrange during a regularly scheduled home visit.

o. Needs to arrange for the pickup of medical equipment or supplies.

p. Has questions or fears about a change in symptoms, such as increased pain or agitation.


REASONS to CALL the HOSPICE TEAM

  1. You are fearful or stressed and need to talk to someone.

  2. You see an unexpected change in the physical or emotional condition of the patient and concerns you.

  3. You observe symptoms such as increased breathing, difficulty, discomfort or agitation.

  4. You have malfunctioning medical equipment.

  5. You have a power failure or other home emergency.

  6. Your loved one passes away.


WHAT HAPPENS WHEN I CALL AFER HOURS?

  1. The nurse will answer your call in 15 minutes to coordinate the care needed, including making a visit.

  2. When the nurse returns your call, explain your concern. You and the nurse will then determine a plan of action.

  3. If your needs are not met to your satisfaction, please request the administrator-on-call.
